NEW PATIENT REGISTRATION

If you wish to join our medical practice, please ask the Receptionist to book an appointment. You will be given a form to complete.

When you attend the appointment, please bring this registration form with you, together with your MEDICAL CARD and a sample of urine.

The Practice does not discriminate on the grounds of race, gender, social class, religion, sexual orientation, appearance, disability or medical condition.

Your rights and responsibilities are set out in our Patient Charter.

Patients who are violent or abusive towards GPs or staff members may be removed from the Practice list. Patients who threaten or are violent towards any GP or staff member will be reported to the Police and further action pursued.

OUT OF HOURS EMERGENCY CALLS

If you are ill and need to contact a Doctor outside normal surgery hours (after 6 pm, weekends or Bank/Public Holidays) please contact SEBDOC, based at Forster Green Hospital, 9079 6220.
