Registration.

new patientsThe doctors welcome new patients who live within our practice area. 

As it often takes sometime for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire and are offered a consultation with a clinician. 

If you wish to join our medical practice, please ask the Receptionist to book an appointment.

You will be given a form to complete.

When you attend the appointment, please bring this registration form with you, together with your MEDICAL CARD, a list of all your medications from your previous GP and a sample of urine.

If you are registering a child you must bring along a list of their previous vaccintions.

The Practice does not discriminate on the grounds of race, gender, social class, religion, sexual orientation, appearance, disability or medical condition.

Your rights and responsibilities are set out in our Patient Charter.

Patients who are violent or abusive towards GPs or staff members may be removed from the Practice list.

Patients who threaten or are violent towards any GP or staff member will be reported to the Police and further action pursued.  

Medical treatment is available from the date of registration.
